1. Click on Forms

2. Click on the form you want to edit

3. Click on Attribution Page

4. Toggle fields on and off

Choose what fields you want on your form

5. Click on Additional Fields

To add your own fields

6. Add your fields

7. Click on Save changes

This adds the field to your form
